Online self-management support

Chronic illness affects many Canadians. People who have the skills, information and confidence to manage the physical and emotional effects of their condition often feel better and can make more effective use of health-care services.

Ontario’s free Online Self-Management Program supports people living with diabetes, chronic pain, high blood pressure, stroke, cancer, heart disease, kidney conditions and other long-term health concerns. Workshops may also help caregivers and people seeking to improve their health literacy.

Prepare for your care

  • Keep an up-to-date medication list.
  • Track symptoms and measurements recommended by your care team.
  • Bring questions and monitoring records to appointments.
  • Discuss realistic goals and barriers with your provider.
